My social security number is 123 45 6789. I was born on July 1 1966. My address is listed above.
I dispute the listing by ABC Collections, Inc on my credit report for account 123456 for a supposed debt to Scamco, Inc. The account should be deleted. I enclosed a copy of my drivers license and a utility bill to show this is my address since I recently moved.
This listing is invalid for many reasons. First, I never agreed to the contract with Scamco and cancelled within the three day rescission period for door-to-door sales. Second, this account is over seven years old as I allegedly owed the debt when demand was made over seven years ago. I enclosed a copy of the original demand from over seven years ago.
I request a description of the procedure used to determine the accuracy and completeness of the information including the name, business address, and telephone number of any person contacted in connection with your investigation.

My name is John Jones and my social security number is and my wife’s name is Alison Jones, social security number. We have been living at the above address for over 2 years.
We have had a lot of problems with a XYZ account no. 12340 that is reporting with false and very damaging information. We need it removed. It is reporting us late for 21 payments. This is false. They have had a lot of trouble determining when the payment is due and the papers we have are confusing to say the least. I have gone over this with various representatives at XYZ and they will not fix this mistake.
The records from XYZ are almost impossible to follow and it seems they are making up certain requirements. We received one notice we were late and called immediately. The representative agreed and removed the late fee. We were surprised in March 2014 to find XYZ was claiming subsequent payments not only were late, but had late fees. We never were sent any bill and the payments were made by automatic deduction. Again, no statement were late was sent and we weren’t late as best I can tell from the confusing, contradictory paperwork XYZ uses.
In your investigation please ask for the original paperwork showing the payment was late and on what day. Please also look over our credit reports and you will see we pay our bills on time and never had a problem paying all our other bills. If we were sent an account statement saying we were late it would have been our highest priority.
